Attention Visitors/Patients of the Cook Hospital and Care Center

Effective 4/11/22:

All Care Center visitors present to the Care Center Entrance where you will self-screen for symptoms and exposure to COVID-19 upon arrival!

Hospital visitors please enter at the Hospital entrance near the ER where you will be screened before entering.

Screening will also be available at the Administration entrance for patients/visitors.

Please perform hand hygiene upon entering. Masks are still required throughout the building.
