MyRecord Patient Portal
We are pleased to announce that the Cook Hospital’s new Patient Portal: MyRecord is now available. This online tool gives you the flexibility to access your health information and other resources on your time and between visits to the Cook Hospital. MyRecord is available over the internet, which means that you can use it from virtually anywhere.
As a patient of the Cook Hospital, enrolling in MyRecord will allow you to:
- Review your Lab Results
- View Medications
- Update Personal Information
- See Visit History and Discharge Information
- View, print, or transmit your Medical Information
- Access a Link to Submit Payments online
Also, MyRecord is completely secure, so you can be confident that your private information is protected. Only you – or an authorized proxy– can access your MyRecord account.
How to enroll for MyRecord
With multiple options, enrollment is easy! You can:
- Print off the MyRecord Patient Portal Request Form or,
- Email myrecord@cookhospital.org and request an enrollment form or,
- Pick up an enrollment form at the registration desk next time you are at the Cook Hospital or,
- Call the Cook Hospital Medical Records department at 218-666-6213 for MyRecord information.
Once you complete your enrollment form, you will be asked to access your email for your one time user name, password and link to the portal.
Click on the link, enter your one time user name, password, and security question and click Log On. Enter your new user name and password.

Where does the health information in MyRecord Patient Portal come from?
All of the information in the Portal comes from the Cook Hospital’s Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.
How do I log into MyRecord?
To log into the Portal, visit the MyRecord link on The Cook Hospital’s website. Then, simply enter your user name and password.
Can my family access MyRecord?
Yes, you can give family members, such as parents or healthcare proxies, access to your Portal. This needs to be done within your healthcare facility and requires consent from you and your family member(s).
Is my information safe?
Yes. Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.
What if I ever have technical problems with the MyRecord Patient Portal?
There is a Contact Us section on the homepage of the Portal that allows you to submit any questions you have regarding the Portal.
