Community COVID Vaccine Information

Information:

The Cook Hospital will be hosting Pfizer Vaccine Clinics for ages 12 and older

Tuesday, May 18^th from 3pm-6pm at the North Woods School (second dose on 6/8)

Thursday, May 20^th from 3pm-6pm at the North Woods School (second dose on 6/10)

Appointments are required, someone from the hospital will call you to set up an appointment.

A parent or legal guardian is required to accompany anyone under the age of 18 to sign appropriate paperwork.
